LIFESPAN COLLABORATIVE SERVICES – BEST PRACTICES

1. Effective January 1, 2018, Public Act 17‐131 Section 3 requires the Electronic Prescribing of all Controlled Substances (EPCS). This means that we will not be able to make paper prescriptions except in certain very limited circumstances.

2. Lifespan Collaborative Services, LLC keeps Protected Health Information (PHI) about members and use this information in the following manner:

3. To coordinate your care among providers, or between a provider and an insurance company;

a. When needed, to ensure that claims are paid to your provider. A release of information may be requested and shared with your providers for that purpose;

b. When the law requires it, we will share PHI ifwe get a court order or ifyour records are subpoenaed (asked for by a court);

c. When we have to collect information about disease or injury. We share PHI to give facts to the CT Public Health Authorities in order to help to prevent a major threat to health or safety.

4. Medication refills where necessary should be requested through the pharmacy, who will then contact the office.

5. A secure electronic health record is utilized to save protected health information and generate prescriptions to the pharmacy of your choice.

6. A secure text messaging and email system is utilized by the electronic health record‐Practice Fusion for appointment confirmations. Data rates may apply to your phone service for text message confirmations.

7. If patients initiate text messaging for questions to the providers' phone; please note that it is not a secure method of communication.
